A rich condition can be caused by fuel pressure too high, leaking fuel injectors, vacuum leak around MAP sensor grommet, oxygen sensor contamination or a restricted exhaust or air intake.

If the front O2S and the rear HO2S voltage reading is greater than 600 mV all the time, the oxygen sensors are probably not at fault.
